Confusion and severe headache in a patient with lupus represent a priority concern for central nervous system involvement or vasculitis, specifically neuropsychiatric lupus. SLE can affect virtually any organ system, and CNS manifestations — including cerebral vasculitis, stroke, seizures, and encephalopathy — are among the most serious complications. When immune complexes deposit in cerebral blood vessels or vasculitis develops in the CNS, patients may present with altered mental status, severe headache, focal neurological deficits, or psychosis. These symptoms demand urgent evaluation including neuroimaging and neurology consultation because delayed treatment can lead to permanent neurological damage or death. Attributing the symptoms to stress would be dangerous, as stress does not cause acute confusion and severe headache. While migraines can produce severe headaches, the addition of confusion is not a typical migraine feature and warrants thorough investigation before assuming a benign cause. Dehydration might cause headache and mild cognitive dulling but would not typically produce the degree of confusion described, and in an SLE patient, CNS lupus or vasculitis must be ruled out first. The nurse must treat this presentation as a neurological emergency until proven otherwise.
